Enter your artist name in the "Artist name" field. If you're logged in, it's already filled in for you. Enter the song title in the "Song title" field. Both will appear in the preview on your image.
If your name or the title is already in the image, tick the matching box: "The artist name is already in the image" or "The song title is already in the image". That way, nothing gets written twice.
